Imaging studies are fundamental in chronic lateral ankle instability (CLAI), especially when physical examination tests are negative in patients with symptoms.
Plain radiographs are used as the initial investigative imaging test, and stress radiographs have a high specificity in actively detecting CLAI. Ultrasound has the advantage of being widely available, with low cost and absence of ionizing radiation. It allows the dynamic assessment of superficial structures, such as tendons and ligaments, with high sensitivity and specificity depending on the examiner. Magnetic Resonance Imaging brings additional information and detailed images of intra-articular and periarticular structures with high accuracy in the setting of CLAI, which is beneficial in preoperative planning. However, a negative MRI study should be interpreted cautiously and not replace arthroscopy, especially in cases with clinical-radiological dissociation. |
