Avoid plantar dissection of the first metatarsal to preserve vascularity. This is especially important in case of avascular necrosis caused by previous osteotomy.
Plan ahead and anticipate first metatarsal shortening. Lesser metatarsal shortening osteotomy is commonly required to maintain the normal metatarsal cascade. Cup and cone reamers are excellent for joint preparation, but care should be practiced to avoid blowing up osteopenic metatarsal heads. Careful radiological and clinical evaluation of the sesamoid-metatarsal joint is strongly recommended, especially in cavus feet, hallux valgus, and revision surgeries. If sesamoidectomy is indicated, the flexor hallucis longus tendon should be preserved. Dorsal plates with a lag screw seem to provide superior biomechanical stability to crossed screws with similar union rates in hallux rigidus. However, in rheumatoid arthritis, hallux valgus, and osteopenic bone in general, dorsal locked plates are recommended. |
