Due to the low success rates of nonoperative management, surgical management of osteochondral lesions of the talus (OLTs) has evolved considerably over the past decade.
Bone-marrow stimulation (BMS) through microfracture or drilling is the most commonly used reparative strategy, especially for smaller, well-circumscribed OLT lesions. In contrast to reparative strategies like microfracture which generate an abnormal fibrocartilaginous articular layer, regenerative strategies attempt to reconstitute normal hyaline cartilage. Complications are common with some surgical management strategies for OLTs, particularly salvage options. Total ankle replacement (TAR) and ankle arthrodesis (AA) are good salvage options in some patients who have developed degenerative changes in the ankle following failed OLT treatment. |
