The etiology of failure of the index ankle fracture open reduction internal fixation should be elucidated by careful review of history, physical examination, plain radiographs, and advanced imaging modalities. Aseptic failure can be attributed to either nonunion, malunion, syndesmotic malreduction, or missed syndesmotic injury. Revision ankle open reduction internal fixation should address the causes of initial failure and may entail extensive debridement, malleolar osteotomy and revision fixation, syndesmosis debridement/revision, reduction/stabilization, and deltoid ligament repair or reconstruction. The use of autograft and biologic adjuvants should be strongly considered especially in cases of nonunion. Postoperatively, frequent and meticulous wound care as well as delayed weightbearing and extended periods of immobilization, should be considered. |
