Tarsal coalition is a congenital skeletal development disorder, with onset of symptoms typically occurring in early adolescence when the coalition starts to ossify. Complex tarsal coalition includes extensive talocalcaneal coalition, double or triple coalition, coalition with severe hindfoot deformities, or coalition with a ball-and-socket ankle deformity.
The presence of severe rigidity associated with a double coalition, a ball-and-socket ankle, or a severe hindfoot deformity are factors that make correction complicated. A thorough physical examination combined with accurate imaging are essential to obtaining detailed information such as the location and size of the coalition, as well as alignment of both the ankle and the hindfoot. Hindfoot deformity present in most pediatric coalition cases can be corrected by an arthroereisis or calcaneus stop technique, calcaneus osteotomies, or hindfoot arthrodesis. In addition to a structural reconstruction, soft tissue and muscle balancing also need to be considered in particular to address any peroneus brevis contracture caused by long-term hindfoot valgus. The ankle joint must be involved in preoperative planning in every case. By doing so, a solid foundation is laid for further surgical planning, which includes coalition resection, stabilization of the foot and ankle, and correcting alignment with osteotomies, arthrodesis, and soft tissue balancing. |
