Background
In this thesis, child abuse cases will be discussed. Medical Imaging Technologists could have an important role in the early detection of abuse. One of the aims of this study is to develop a guide to good practice for MITs in detecting warning signs of non-accidental trauma. This guide will contain information on how to help detect signs of non-accidental trauma through observation when caring for pediatric patients (infants, children and adolescents), as well as guidelines on how to behave in the event of suspicion and to whom the report of a non-accidental trauma situation should be made. One of the purposes of this paper is to gradually reach a situation where these proactive attitudes towards suspected non-accidental trauma will become known to all stakeholders and will be applied, and the cases can then be dealt with more quickly and efficiently. Another point of this work concerns the administrative and legal steps to be taken after suspecting a case of mistreatment. This means knowing the limits of the MIT in this area, whether or not it can go further in the case of suspected non-accidental trauma. And also knowing who they should talk to between the supervising radiologist, the emergency doctor, the prescribing doctor or referring clinician, a paramedical officer, or others. Materials & Methods The study will take place at the Cliniques Universitaires de Saint-Luc in the pediatric radiology department. The internship will take place between 8 February and 26 February 2021. It is important to specify that the number of cases of non-accidental trauma is very limited, even zero, taking into account its rarity. For this reason of rarity, an agreement with the pediatric emergency department is obtained to be notified in case a non-accidental trauma occurs after the internship period. All paramedical staff, mainly radiologists and staff of the "SOS Children" services on all the points to know when medical imaging technologists are faced with a non-accidental trauma situation. A survey dedicated to paramedical staff working in the Medical Imaging department will be established in various hospitals in Brussels and Wallonia. This survey has been created to demonstrate the need for a help booklet for the MIT confronted with a suspicion of non-accidental trauma. The list of questions follows a structure that focuses on how staff feel when faced with non-accidental trauma. And it assesses their approaches to detect the various signs of non-accidental trauma. Results The results show that there is a lack of information on the indirect signs of non-accidental trauma and therefore justifies the need for an help brochure for the medical imaging technologist confronted with a suspicion of non-accidental trauma in children during an imaging examination. Conclusions The main objective is to develop a guide for medical imaging technologists on how to detect the warning signs of non-accidental trauma. The aim is to increase awareness among health professionals, who are often on the front line, of how to detect and report them when appropriate. The different signs that will alert a medical imaging technologist are therefore the different typical fractures related to non-accidental trauma, the skin lesions that can be observed in the child in specific areas of the body, and the observation of the child's reaction with the parents and vice versa. It is also important to check that the lesion is consistent with the reason for the examination. The contact person who is in the first line of a suspicion of non-accidental trauma in a child is the supervising radiologist, but also the prescribing physician and/or the paramedic. The technologist may also make a referral to the "SOS Children" services. The limits of the technologist are that he/she cannot tell the parents about the suspicion of non-accidental trauma, this is the responsibility of the physician. |
