Medial ankle instability is mainly a clinical diagnosis.
In an acute injury, tenderness, ecchymosis, and swelling over the deltoid ligament have relatively poor sensitivity. In a chronic condition, pain on palpation at anteromedial edge and a valgus and pronation deformity of the loaded foot, which is seen to disappear when the patient is asked to activate the posterior tibial muscle or to go in tiptoe position, are the hallmark for the presence of medial ankle instability. Various stress tests permit to identify specific injury patterns of the deltoid and spring ligaments and to elucidate an associated dysfunction of the posterior tibial tendon. A hyperactivity of the flexor hallucis longus muscle is typically initiated, which results in an elevation of the first metatarsal head with the appearance of a pseudo hallux rigidus. |
