Résumé : | The gastrocnemius flap is a versatile flap when muscle or muscle and skin are desired for coverage of bone of the proximal one-third of the leg and about the knee. Both the medial and lateral heads, or both combined, may be used as flaps. The medial gastrocnemius has a greater excursion, mass, and perforator distribution.1, 2, 3, 4 Typically, the reach of only the muscle restricts reach to just below the knee. Release off the medial femoral condyle permits increased reach, and a bit more is obtained by scoring the deep fascia and gently slowing expansion of the muscle. When a skin island is added (myocutaneous medial gastrocnemius flap), however, the reach can be extended both proximally over the knee and distally over the anterior leg. The lateral head is not as reliable to support a skin island. |
