Zone 1 fractures have excellent healing potential and may be treated nonoperatively with a weightbearing as tolerated protocol.
Jones fractures may refer to proximal fifth metatarsal fractures in zones 2 and 3. Zone 2 fractures are truly acute injuries, whereas zone 3 fractures commonly occur in the presence of a chronic stress reaction. Zone 2 fractures may heal well with nonoperative or operative treatment. Zone 3 fractures typically require operative treatment. Axial screw fixation is an effective operative treatment of Jones fractures, but the optimal screw design remains unclear. Bone grafting and bone marrow aspirate concentrate injection are emerging adjuvant treatments for patients with zone 2 or zone 3 injuries at high risk for nonunion or refracture. |
