Up to 45% of surgically treated ankle fractures have associated syndesmotic disruption. Quality of syndesmotic reduction appears to affect functional outcomes.
Syndesmotic malreduction ranges from 15% to 50% in the literature, and achieving anatomic reduction remains a significant challenge. Certain ankle fractures are more prone to malreduction: pronation injuries, fracture-dislocations, comminuted fibula fractures, and trimalleolar ankle fractures. Preoperative computed tomography scan can identify a posterior malleolus fracture or syndesmotic anatomy that may predispose to malreduction. It is essential to have a stepwise plan and an understanding of reliable fluoroscopic parameters for evaluating the reduction in both coronal and sagittal planes intraoperatively. |
