The deltoid ligament complex stabilizes the ankle joint against eversion, external rotation, and plantar flexion forces.
There is no consensus on whether deltoid ligament must be repaired in bimalleolar equivalent ankle fractures. Patients with extensive capsuloligamentous damage and multidirectional instability may benefit from repair to improve reduction quality, avoid loss of reduction, and regain medial/syndesmotic stability. Most deltoid ligament repair techniques involve direct repair or reattachment using suture anchors for augmentation. There is no evidence of the superiority of any technique in the literature. The authors recommend repairing the deltoid ligament complex in the setting of bimalleolar equivalent fractures associated with syndesmotic or multiligamentous instability as well as in heavier patients with greater mechanical requirements. |
