The aim of this study is to validate a platelet aggregation method on a broad automated coagulation machine, the Sysmex CS-2500 by comparing it to a reference manual machine, the PAP-8 (Platelet Aggregation Profiler). The CS-2500 has several advantages; it is simple, fast and offers automation to minimize the inter-operator risk, on the contrary the PAP-8 is a device requiring manual manipulation, and requiring experienced technology.
The test makes it possible to diagnose disorders of primary hemostasis, it studies the anomalies of platelet function which can be constitutional or acquired. A total of 26 samples were collected on citrate tube thanks to the healthy candidates. The method is based on a photometric measurement, between a light source and a detector is placed a platelet-rich plasma (PRP) which is obtained by low centrifugation, platelet activators are added to the latter in order to cause their aggregation. The PRP is then centrifuged at low speed to obtain a platelet-poor plasma (PPP), it is used to calibrate the machine. In summary, this test assesses the variation in the percentage of light transmitted as a function of time, describing this phenomenon of platelet aggregation. The results are satisfactory for setting the standards on the CS-2500 because the two methods are consistent, moreover it is no longer necessary to dilute the plasma with PPP which was not recommended. However, regarding the performance of the automat, the repeatability was good for all agonists, but the limit of quantification was not as low as described in the literature or by the firm for any of the agonists. In conclusion, the CS-2500 is a promising automat in the diagnosis of platelet anomalies, however, to use the machine routinely, it is necessary to assess the symptomatic population and to assess the limit of quantification on more healthy patients (n=5) for a correct interpretation of the results. |
