75% of Talar avascular necrosis (AVN) is associated with trauma, and in particular, talar neck fractures.
Revascularization of the talus can take up to 36 months by creeping substitution. Prolonged nonweight bearing, partial weight bearing, and bracing have been used for conservative treatment but have not been shown to prevent collapse or improve outcomes. Tibiotalar arthrodesis, subtalar arthrodesis, and tibiotalocalcaneal arthrodesis are all potential options for treating late-stage talar AVN based on patient examination, imaging, and selective joint injections. Principles of arthrodesis include the removal of any necrotic bone; the addition of autograft, allograft, and various biologic augments for incorporation; and instrumentation that maintains appropriate rigidity, compression, and alignment. |
