Müller-Weiss disease (MWD) is a dysplasia of the tarsal navicular that is developed during childhood and suffered during adulthood.
Pathomechanics in MWD include the shifting of the talar head laterally over the os calcis, thus driving the subtalar joint into varus. Failure to identify patients with paradoxic flatfeet (with hindfoot varus instead of conventional valgus) may lead to incorrect diagnosis and management. Conservative treatment with the use of rigid insoles with medial arch support and a lateral heel wedge to reduce supination of the heel is effective in most patients. Dwyer calcaneal osteotomy combined with lateral displacement seems to be a satisfactory treatment for the management of patients with MWD that had failed to respond to conservative measures. Joint-preserving surgery seems a good alternative to the different types of perinavicular fusions that do not address pathomechanics of MWD. |
