Étude de la faisabilité, de la reproductibilité et de la validité externe versus critères de référence des paramètres échographiques de longle dans le rhumatisme psoriasique.
Méthodes
Étude pilote prospective monocentrique sur huit paramètres échographiques en mode B et doppler puissance portant sur linterphalangienne distale (IPD), la matrice, le lit et la tablette unguéale. La reproductibilité intra-observateur et inter-observateur a été évaluée pour les sept paramètres quantitatifs (ICC et kappa). La recherche de corrélations entre les variables échographiques et cliniques a permis destimer la validité externe. La faisabilité a été appréciée par le temps dexamen et le pourcentage de données manquantes.
Résultats
Vingt-sept patients (âge : 55,0+16,2ans, durée de la maladie : 13,4+9,4ans) ont été inclus dont 67 % présentaient une atteinte échographique unguéale vs 37 % clinique (p 0,75) pour les sept paramètres quantitatifs, sauf pour la synovite de lIPD en mode B. Les synovites échographiques des IPD étaient corrélées aux nombres de synovites cliniques et au doppler de longle (matrice et lit). Le doppler de la matrice était corrélé à lEVA douleur mais pas à lASDAS-CRP, ni aux enthésites cliniques. Aucune corrélation na été observée avec les épaisseurs de longle.
Conclusion
La faisabilité et la reproductibilité de léchographie de la région unguéale dans le rhumatisme psoriasique sont bonnes. Parmi les 8 paramètres évalués, le doppler de la matrice corrélé avec linflammation locale (IPD et lit) et à lEVA douleur pourrait être un critère de suivi intéressant, sous réserve dêtre sensible au changement.Accès : Article disponible sur EM-Premium via le proxy de la HE Vinci Disponible en ligne : Oui Permalink : https://bib.vinci.be/opac_css/index.php?lvl=notice_display&id=135133

The goal was to examine the feasibility of a randomized controlled trial (RCT) on the effect of a therapeutic foot-ankle training program to prevent injury in long-distance runners. First, we evaluated (i) the access to participants and recruitment success; (ii) participants' satisfaction and adherence to the program; (iii) the effect of the training program to improve foot muscle strength and change foot biomechanics; and, second, we used the collected data for a post hoc sample size calculation.
Methods/design
We randomized 31 healthy long-distance recreational runners to either an 8-week foot-ankle muscle strength-training program (intervention) or a stretching protocol (control). The recruitment rate was the number of eligible participants per week of recruitment; recruitment success, the ratio between scheduled baseline visits and initially eligible participants. Participant satisfaction was assessed by a questionnaire, and adherence to the training program was recorded in a Web-based software, both at the 8-week mark. Program effect was assessed by hallux and toe muscle strength using a pressure platform, foot muscle cross-sectional area using magnetic resonance imaging and foot kinematics during running using 3D gait analysis; assessments were done at baseline and after 8 and 16 weeks. A post hoc power analysis was performed on foot strength and the biomechanical data was collected.
Results
In two weeks of recruitment, 112 initially eligible subjects were screened, 81 of whom were deemed eligible and 31 had a baseline study visit, giving a recruitment rate of 40.5 subjects/week and recruitment success of 28%. Participants' adherence was 97%, and satisfaction scored a median >3 out of 5 on a Likert scale on all questions. The cross-sectional area of the abductor hallucis (P = 0.040) and flexor digitorum brevis (P = 0.045) increased significantly at 8 weeks in the intervention group. The post hoc sample sizes for almost all the strength and biomechanical parameters were below those of the 112 subjects calculated as the original sample size for clinical outcome (running-related injury).
Conclusion
Results show that this RCT is feasible, given an accessible study population that is willing to participate and that perceives the training program as positive and adheres to the program. Living with a melanoma diagnosis can be challenging. We aimed to assess the feasibility, acceptability, and perceived value of a nurse-led intervention that utilised patient-reported outcome (PRO) measures to identify and address the supportive care needs of newly diagnosed patients with Stage I/II melanoma over the first 4 months post-diagnosis.
Methods
We conducted an exploratory, repeated-measures, single-arm, feasibility trial. One baseline (4 weeks post-diagnosis; T1) and one follow-up intervention session (4 weeks after wide local excision; T3) took place, two months apart. Patient survey data were collected monthly, at four assessment points (T1-T4), followed by exit interviews.
Results
A recruitment rate of 55% (10/18) was achieved. The skin cancer nurse specialist (CNS) performed 19 in-clinic patient assessments within 6 months. One patient missed their follow-up intervention session (90% retention rate). Three participants (30%) were lost to follow-up at T4. Patients endorsed the standardised use of easy-to-use PRO measures as a means to help them shortlist, report and prioritise their needs. The CNS viewed the intervention as a highly structured activity that allowed tailoring support priority needs. The intervention appears to be feasible in clinical practice and acceptable to both patients with newly diagnosed melanoma and clinicians. This study sought to test the acceptability and feasibility of a nurse-led psycho-educational intervention (NLPI) delivered in primary care to prostate cancer survivors, and to provide preliminary estimates of the effectiveness of the intervention.
Methods
Men who reported an ongoing problem with urinary, bowel, sexual or hormone-related functioning/vitality on a self-completion questionnaire were invited to participate. Participants were randomly assigned to the NLPI plus usual care, or to usual care alone. Recruitment and retention rates were assessed. Prostate-related quality of life, self-efficacy, unmet needs, and psychological morbidity were measured at baseline and 9 months. Health-care resource use data was also collected. An integrated qualitative study assessed experiences of the intervention.
Results
61% eligible men (83/136) participated in the trial, with an 87% (72/83) completion rate. Interviews indicated that the intervention filled an important gap in care following treatment completion, helping men to self-manage, and improving their sense of well-being. However, only a small reduction in unmet needs and small improvement in self-efficacy was observed, and no difference in prostate-related quality of life or psychological morbidity. Patients receiving the NLPI recorded more primary care visits, while the usual care group recorded more secondary care visits. Most men (70%; (21/30)) felt the optimal time for the intervention was around the time of diagnosis/before the end of treatment.
Conclusions
