Aides auditives ; Présentations de cas ; Recueil de l'anamnèseRésumé : Le bilan audiométrique est réalisable et les mesures liminaires
et supraliminaires ont mis en évidence une surdité bilatérale
moyenne légèrement asymétrique en défaveur de l’oreille
droite avec une dynamique réduite. La perte auditive moyenne
est de 69 dB à droite et de 61 dB à gauche (Figure 1).
Les tests vocaux ont également pu être réalisés. Ils indiquent
une amélioration par l’amplification linéaire de l’audiomètre
mais avec des scores limités. Sur les listes cochléaires de Lafon,
le maximum d’intelligibilité (88%) est obtenu en modalité
binaurale à 105 dB. (90 % à 110 dB pour l’oreille droite et 86%
à 110 dB pour l’oreille gauche).
On notera également une grande variabilité des performances
selon le matériel vocal utilisé (mots vs phrases). Les résultats
pouvant être dus à la grande fatigabilité du patient.
A l’issue du bilan, compte-tenu des résultats, le patient et sa
famille ont été informés sur les limites de l’appareillage et le
bénéfice potentiellement attendu. Un avis orthophonique
complémentaire a été préconisé. Un essai d’appareillage
stéréophonique avec des appareils de classe 1 a été décidé.
Des objectifs précis ont été fixés pour le patient :
- Faciliter la fluidité des échanges pendant les visites de la famille
- Améliorer la compréhension au moment des repas
- Favoriser la participation et les interactions lors des activités

To investigate the diagnostic accuracy of patient history associated with hip pain.
Data Sources
A systematic, computerized search of electronic databases (PubMed, MEDLINE, Cumulative Index of Nursing and Allied Health Literature, and Web of Science), a search of the gray literature, and review of the primary author's personal library was performed. Hip-specific search terms were combined with diagnostic accuracy and subjective or self-report history-based search terms using the Boolean operator “AND.”
Study Selection
This systematic review was conducted and reported according to the protocol outlined by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The inclusion criteria were: (1) patients with hip pain; (2) the statistical association of at least 1 patient history item was reported; (3) study designs appropriate for diagnostic accuracy; (4) adults aged ≥18 years; (5) written in English; and (6) used an acceptable reference standard for diagnosed hip pathology. Titles and abstracts of all database-captured citations were independently screened by at least 2 reviewers.
Data Extraction
Two reviewers independently extracted information and data regarding author, year, study population, study design, criterion standard, and strength of association statistics associated with the subjective findings.
Data Synthesis
For hip osteoarthritis (OA), a family history of OA (positive likelihood ratio [+LR], 2.13), history of knee OA (+LR, 2.06), report of groin or anterior thigh pain (+LR, 2.51-3.86), self-reported limitation in range of motion of 1 or both hips (+LR, 2.87), constant low back pain or buttock pain (+LR, 6.50), groin pain on the same side (+LR, 3.63), and a screening questionnaire (+LR, 3.87-13.29) were the most significant findings. For intra-articular hip pathology, crepitus (+LR, 3.56) was the most significant finding.
Conclusions

To investigate whether a family history of low back pain (LBP) influences patient outcomes and treatment effects following home exercises in older people with chronic LBP.
Design
Secondary analysis of a randomized controlled trial.
Setting
Local community.
Participants
People older than 55 years with chronic LBP (N=60).
Interventions
Participants in the intervention group completed video game exercises for 60 minutes 3 times per week for 8 weeks. Participants in the control group were instructed to maintain their usual levels of activity and care seeking behaviors.
Main Outcomes Measures
Participants indicated whether any of their immediate family members had a history of “any” LBP or “activity-limiting” LBP at baseline. We collected self-reported measures of pain, function, pain self-efficacy, care seeking, physical activity, disability, fear of movement and/or reinjury, and falls efficacy at baseline, 8 weeks, 3 months, and 6 months. We performed regression analyses to determine whether a family history of LBP predicted patient outcomes and moderated the effects of home exercise.
Results
Participants with a family history of any LBP were less likely to be highly active than those without a family history (odds ratio, 0.08; 95% CI, 0.01-0.42; P=.003). Home-based video game exercises led to improvements in function in those without a family history of activity-limiting LBP (β=1.78; 95% CI, 0.56-3.00; P=.006) but not in those with a family history (β=−0.17; 95% CI, −2.56 to 2.21; P=.880) (interaction P=.049). A family history of LBP did not influence the remaining patient outcomes or treatment effects.
Conclusions
A family history of LBP appears to negatively influence physical activity levels in older people with chronic LBP.
