The purpose of this study was to examine the association between pain sensitivity, central sensitization, and functional disability in adolescents with joint hypermobility.
Design and Methods
A cross-sectional descriptive design was utilized for this study. A sample of 40 adolescents being evaluated for chronic pain and autonomic nervous system dysfunction were recruited. Subjects were evaluated for pain, function, central sensitization, and sensitivity on pain and touch sensory nerve fiber types. Data were analyzed to detect associations between variables.
Results
Joint hypermobility had a moderately significant correlation with central sensitization measured by the Central Sensitization Inventory, as well as increased pain sensitivity as evidenced by hypersensitivity of Aδ sensory nerve fibers. The presence of central sensitization was also positively associated with level of functional disability.
Conclusions
Findings implicate joint hypermobility as a possible antecedent to pain hypersensitivity and central sensitization syndromes that when recognized and addressed effectively may reduce functional disability in those affected.
Practice Implications
It is important for pediatric nurses that work with adolescents that have chronic pain to understand variables that may impact pain and functional disability in order to develop methods to increase function, reduce pain, and increase perceived quality of life.Disponible en ligne : Non Permalink : https://bib.vinci.be/opac_css/index.php?lvl=notice_display&id=86233

To describe outcomes of nonsurgical treatment for symptomatic thumb carpometacarpal joint (CMC-1) instability. Secondary, to evaluate the conversion rate to surgical treatment.
Design
Prospective cohort study.
Setting
A total of 20 outpatient clinics for hand surgery and hand therapy in the Netherlands.
Participants
A consecutive sample of patients with symptomatic CMC-1 instability (N=431).
Intervention
Nonsurgical treatment including exercise therapy and an orthosis.
Main Outcome Measures
Pain (visual analog scale [VAS], 0-100) and hand function (Michigan Hand Outcomes Questionnaire [MHQ], 0-100) at baseline, 6 weeks, and 3 months. Conversion to surgery was recorded for all patients with a median follow-up of 2.8 years (range, 0.8-6.7y).
Results
VAS scores for pain during the last week, at rest, and during physical load improved with a mean difference at 3 months of 17 (97.5% CI, 9-25), 13 (97.5% CI, 9-18), and 19 (97.5% CI, 12-27), respectively (P<.001 no difference was present at months for mhq total score but the subscales activities of daily living work pain and satisfaction improved by ci points respectively after median follow-up years only participants were surgically treated. both in subgroups that did not convert to surgery vas scores decreased compared with baseline whereas improve subgroups. however remained worse patients who eventually converted> Conclusions
In this large sample of patients with symptomatic CMC-1 instability, nonsurgical treatment demonstrated clinically relevant improvements in pain and aspects of hand function. To (1) establish the association of the most common reported symptoms on disability; and (2) study the effectiveness of treatment on disability in patients with Ehlers-Danlos syndromehypermobility type (EDS-HT)/hypermobility syndrome (HMS).
Data Sources
An electronic search (Medical Subject Headings and free-text terms) was conducted in bibliographic databases CENTRAL/MEDLINE.
Study Selection
Comparative, cross-sectional, longitudinal cohort studies and (randomized) controlled trials including patients with HMS/EDS-HT aged ≥17 years were considered for inclusion. A class of symptoms was included when 5 publications were available. In regards to treatment (physical, cognitive interventions), only (randomized) controlled trials were considered. Surgical and medicinal interventions were excluded.
Data Extraction
Bias was assessed according to the methodological scoring tools of the Cochrane collaboration. Z-score transformations were applied to classify the extent of disability in comparison with healthy controls and to ensure comparability between studies.
Data Synthesis
Initially, the electronic search yielded 714 publications, and 21 articles remained for analysis after selection. The following symptoms were included for meta-analysis: pain (n=12), fatigue (n=6), and psychological distress (n=7). Pain (r=.64, P=.021), fatigue (r=.91, P=.011), and psychological distress (r=.86, P=.018) had a significant impact on disability. Regarding treatment, a significant pain reduction was achieved by a variety of physical and cognitive approaches. Treatment effectiveness on disability was not established.
Conclusions
Disability can affect patients with HMS/EDS-HT significantly and is highly correlated with both physical and psychological factors. Comparer le profil douloureux et psychosocial des femmes ayant un Syndrome dEhlers-Danlos Hypermobile (SEDh) et une Polyarthrite Rhumatoïde (PR).
Méthode
Dix-neuf femmes atteintes de SEDh et vingt-six de PR recrutées par le biais des associations et groupes de patients ont rempli lInventaire Multidimensionnel de la Douleur. Les zones douloureuses, lâge dapparition de symptômes et celui au moment du diagnostic ont été également récoltés.
Résultats
Des différences significatives ont été observées entre les groupes concernant lâge dapparition de symptômes (SEDh=12 ans vs. PR=30 ans) ainsi quau niveau du retard diagnostic (SEDh=20 ans vs. PR=1 an). Le nombre de régions douloureuses, la sévérité de la douleur et la détresse émotionnelle sont supérieurs dans le groupe SEDh. Enfin, le groupe PR réalise significativement plus dactivités générales et de loisirs que le groupe SEDh.
Conclusion
Malgré ses limites, cette étude suggère que les femmes avec un SEDh semblent particulièrement vulnérables sur certains aspects physiques et psychosociaux. Des efforts sont nécessaires afin de réduire le retard diagnostic dans le SEDh et minimiser les conséquences négatives de la pathologie.

Summary
Purpose
To compare the pain and psychosocial profile of women with hypermobile Ehlers-Danlos Syndrome (hEDS) to that of Rheumatoid Arthritis (RA).
Method
Nineteen women with hEDS and twenty-six with RA recruited through patients associations and groups completed the Multidimensional Pain Inventory. Pain areas, age of symptom onset and age at diagnosis were also collected.
Results
Significant differences were observed between groups in the age of symptom onset (hEDS=12 years vs. RA=30 years) and in diagnosis delay (hEDS=20 years vs. RA=1 year). Number of painful areas, severity of pain and emotional distress are higher in the hEDS group. Finally, the RA group performs significantly more general and leisure activities than the SEDh group.
Conclusion
Despite its limitations, this study suggests that women with hEDS appear to be particularly vulnerable in certain physical and psychosocial aspects.
