Diagnostic auditif ; Neuropathie auditive (NA) ; Perte d'auditionRésumé : Objectives: The aim of the study was to review the demographic and clinical characteristics of all pediatric patients diagnosed with auditory neuropathy spectrum disorder (ANSD) by a pediatric health care system from 2005 to 2020 and examine whether or not our diagnostic capabilities in an ANSD population have evolved as our institutional experience has grown and knowledge in the field has expanded.
Design: This was a retrospective study reviewing the demographic data, medical history, imaging studies, audiological and speech-language data, type of audiological intervention and mode of communication in 260 pediatric patients diagnosed with ANSD over a 15-year period.
Results: The study revealed that male and female children were equally affected with all levels of hearing detection being represented and that about 40% of affected children were premature and most were admitted to the neonatal intensive care unit. More than a third of our patients presented with a complex medical history and/or neural involvement while about 30% were full-term newborns with normal pregnancy, no prenatal complications or infections, normal birth weight, no neonatal intensive care unit need, no hyperbilirubinemia, no respiratory distress requiring ventilation, and no known syndrome. Review of audiological findings confirms that otoacoustic emissions are not always present in ANSD cases, and that the presence of an abnormal wave V on the auditory brainstem response tracings (only present at high intensities and with an absent intensity/latency function) is not a rare finding and should not immediately be dismissed as not being a case of ANSD.
Conclusions: This review of ANSD diagnosis over a 15-year period clearly reveals the drastic improvements made in the identification of ANSD, with a drastic decrease in the age at diagnosis and a reduction in the percentage of misdiagnosed patients. Individuals who are deaf or hard of hearing (DHH) may find adherence to their hearing devices difficult due to internal experiences related to their hearing loss such as sadness or frustration. The Acceptance and Action Questionnaire-Adult Hearing Loss (AAQ-AHL) is the only measure available to assess psychological inflexibility as it relates to hearing loss. The purpose of this study was to confirm the single latent structure of the AAQ-AHL (through confirmatory factory analysis) and test convergent and discriminant validity.
Design
Cross-sectional data was used to further validate the AAQ-AHL.
Study sample
Participants were 146 adults who had diagnosed hearing loss and used a hearing aid.
Results
Results revealed the AAQ-AHL has a single latent structure, correlated to other similar constructs (psychological flexibility and hearing aid efficacy), and not correlated to unrelated constructs (hearing loss severity). These results suggest that the AAQ-AHL is a valid instrument to assess psychological flexibility as it relates to hearing aid use.
Conclusion

Dépistage auditif ; Internet ; Perte d'audition ; Rééducation auditive ; Sujet âgéRésumé : Objectif : Cette étude a mesuré l'état de préparation à la recherche d'aide et l'acceptation des sites Web existants de soins auditifs sur Internet (IHHC) parmi un groupe de personnes âgées qui ont échoué à un dépistage auditif (phase 1). Il a également exploré les effets d'une brève formation sur l'acceptation par les participants de l'IHHC (Phase 2). Échantillon de l'étude : 27 adultes (55 ans et plus) qui ont échoué à un dépistage auditif ont participé. ConceptionAu cours de la phase 1, les participants ont reçu le questionnaire d'évaluation du changement de l'Université du Rhode Island (URICA) et le modèle PTAM (Patient Technology Accepting Model). Pendant la phase 2, les participants ont été assignés au hasard à un groupe de formation ou de contrôle. Les participants du groupe de formation ont assisté à une classe d'enseignement sur les sites Web existants de l'IHHC. Le groupe témoin n'a reçu aucune formation. Le questionnaire PTAM a été ré-administré aux deux groupes 4 à 6 semaines après l'évaluation initiale. Résultats : La majorité des participants envisageaient ou se préparaient à faire quelque chose au sujet de leur perte auditive et acceptaient généralement les sites Web de l'IHHC (phase 1). Les participants qui ont suivi une brève formation de l'IHHC ont signalé une augmentation des connaissances en matière de santé auditive et de légères améliorations de l'auto-efficacité informatique (phase 2).Conclusion s: Les adultes plus âgés qui ne parviennent pas à entendre les dépistages peuvent être de bons candidats pour l'IHHC. L'incorporation d'une interface utilisateur simple et d'une formation à court terme pourrait optimiser l'utilisabilité des futurs programmes de PSSI pour cette population. There is an increased interest in the impact that hearing loss has on general well-being, including overall quality of life (QOL), to improve and expand care that is provided to individuals with hearing loss.
Purpose
To evaluate QOL in adults with and without access to hearing health care (HHC).
Research Design
A cross-sectional study examined QOL across groups of individuals with and without hearing loss.
Study Sample
One hundred eight participants from West Central and South Alabama received pure-tone hearing evaluations. Thirty-two adults had hearing within normal limits and 76 had at least a mild hearing loss in one ear.
Data Collection and Analysis
The Quality of Life Inventory (QOLI), the Charlson Comorbidity Index, and an Accessibility to Health Care questionnaire were administered to all participants. The QOLI outcomes were used as the dependent variable for the analysis of covariance (ANCOVA) statistical procedures.
Results
For adults with hearing loss who did not have access to HHC, lower QOL scores were reported compared with those with access to HHC, but this finding was not significant. Although ANCOVA results did not suggest QOL differences across geographical regions, effect size calculations indicated that adults with hearing loss who lived in the most rural regions of Alabama had lower reported QOL scores than their counterparts who had hearing within normal limits. Finally, those with higher incomes, who were older, and who had fewer physical disorders reported higher QOL than those with lower incomes, who were younger, and who had more physical ailments.
Conclusions
Adults with hearing loss who live in regions without access to HHC might be at risk for decreased QOL.
