Auto-Hypnose ; Douleur ; Hypnose ; Nociception ; Perception de la douleur ; Peur ; Sage-femme ; Théorie ; Travail obstétricalRésumé : Pain and fear are high on the list of reasons that women give for seeking hospital admission when they are in labour (Cheyne et al 2007, Barnett et al 2008, Carlson et al 2009, Carlsson 2016).
The timing of admission to hospital is important because women experiencing an uncomplicated pregnancy are at increased risk of obstetric intervention if they are admitted to hospital during early labour (Bailit et al 2005, Lundgren et al 2013, Neal et al 2014). This knowledge causes many women, who might have benefited from professional psychological support, to be sent home and left to manage this period of labour alone (Barnett et al 2008, Eri et al 2015). More work is needed to understand how women can be effectively supported in managing their pain at this time and safely await active labour before coming to hospital (Eri et al 2015, Kobayashi et al 2017).
This paper is presented as one of two papers aimed at prompting our thinking and understanding around pain perception in labour. It provides a summary of our current understanding about pain, and highlights pain catastrophising and how it might affect childbirth. Indirect evaluation tests of somatosensory integrity, such as the left/right judgment task and two-point discrimination thresholds, are used to complement a clinical evaluation facilitating tailored rehabilitation programs for patients with chronic pain. This study aimed to compare performance on the left/right judgment task and two-point shoulder discrimination thresholds between people with chronic shoulder pain and asymptomatic controls. The study's secondary objective was to investigate whether there is a relationship between pain intensity and the painful area with TPDT and LRJT.
Materials and methods:
It is a cross-sectional study with a control group. Volunteers participated in the left/right judgment task using the Recognise ShoulderTM app, and two-point discrimination thresholds were performed using a Mitutoyo® digital caliper. The extent of the painful area, affectivity, and functionality were also determined to characterize the sample.
Results:
We assessed 52 symptomatic and 50 asymptomatic individuals. A between-group difference was observed in time response for the left/right judgment task (p = 0.025) and the two-point discrimination thresholds when tested over the shoulder's lateral region (p = 0.012). There was a significant positive weak correlation between pain intensity and TPDT test performance over the anterior shoulder (rs = 0.35; p = 0.01) and a positive weak correlation between pain area and LRJT response time (rs = 0.26; p = 0.04).
Conclusions:

This study was performed to compare the effects of a vapocoolant spray and a eutectic mixture of local anesthetics (EMLA) cream on reducing pain during intra-articular (IA) injection of the shoulder.
Design
Double-blind randomized placebo-controlled clinical trial.
Setting
University hospital.
Participants
Patients (N=63) who underwent IA injection of the shoulder joint were randomized into the spray group, EMLA group, or placebo group.
Intervention
Placebo cream+vapocoolant spray (spray group), EMLA cream+placebo spray (EMLA group), or placebo cream+placebo spray (placebo group) before IA injection.
Main Outcome Measures
A 100-mm visual analog scale (VAS) for injection pain and 5-point Likert scales for participant satisfaction and preference for repeated use were administered immediately after IA injection.
Results
The VAS scores for pain during IA injection were 30.0 (95% CI, 19.7-41.2) in the spray group, 50.0 (95% CI, 37.7-63.0) in the EMLA group, and 53.8 (95% CI, 41.6-65.0) in the placebo group (F=6.403, P<.01 the spray group showed significantly better likert scale scores than placebo for participant satisfaction and preference repeated use> Conclusions

To compare the effects of a vapocoolant spray and an eutectic mixture of local anesthetics (EMLA) cream in reducing pain during needle electromyography examination.
Design
Randomized controlled trial.
Setting
Physical medicine and rehabilitation department of a university hospital.
Participants
Adults who underwent needle electromyography (N=99) were randomized to 1 of 2 experimental groups or the control group. Two patients dropped out during the study.
Interventions
In the experimental groups, vapocoolant spray or EMLA cream were applied before needle electromyography. In the control group, needle electromyography was performed without pretreatment.
Main Outcome Measures
Intensity of pain associated with needle electromyography was assessed using a 100-mm visual analog scale (VAS). Patient satisfaction and preference for repeated use were measured using a 5-point Likert scale.
Results
VAS score for pain intensity was significantly lower in the spray group (31.9; 95% confidence interval [CI], 22.041.7) compared with the control group (52.9; 95% CI, 45.960.0; P=.002), whereas there was no significant difference between the EMLA cream group (42.4; 95% CI, 34.250.7) and the control group. Patient satisfaction and preference for repeated use were higher in the spray group than the EMLA group.
Conclusions
