High pain intensity at time of admission for injury is a risk factor for chronic pain, but it is not clear whether specific acute pain patterns following injury influence the development of chronic pain. To examine the relationship between the pain trajectory, the mean pain score, and the frequency of pain documentation during the immediate hospitalization following injury, with the report of chronic pain. This was a descriptive, retrospective cohort study of adults admitted with lower extremity fractures to an academic urban trauma center. Participants, 6-45 months postinjury, rated their current pain, worst pain, and average pain over the last 3 months. Pain scores from hospitalization associated with the injury were obtained through a retrospective chart review. The pain trajectory, mean pain score, and frequency of pain documentation was compared between patients with and without chronic pain. A total of 129 patients were enrolled in this study and 78% reported chronic pain at the site of injury. The mean pain score (5.1 vs. 4.2) and first pain score (5.6 vs. 3.4) were higher for patients with chronic pain compared to patients with no chronic pain. Consistent with other studies, high pain intensity at time of injury was associated with chronic pain. The findings contribute valuable information about acute pain characteristics associated with chronic pain and provide insight into the importance of early and adequate acute pain treatment. The purpose of this study was to assess the association of adolescent and parent psychological beliefs with 1) self-reported functional ability, 2) pain and 3) objective measures of function.
Study design
Cross-sectional study.
Setting
Pediatric Outpatient Hospital.
Participants
Eighty-six adolescents with patellofemoral pain (PFP) (14.6 ± 1.7 years old, 62% female) and 72 parents.
Main outcome measures
Patient questionnaires were used to describe pain, knee function, fear avoidance (Fear Avoidance Beliefs Questionnaire-Physical Activity; FABQ-PA), kinesiophobia (Tampa Scale for Kinesiophobia-11; TSK-11), and pain catastrophizing (Pain Catastrophizing Scale; PCS) in adolescents with PFP. Parents (n = 72) completed FABQ-PA, TSK-11, and PCS questionnaires. Hip and knee strength, quadriceps and dorsiflexion motion, the single-leg hop for distance and lateral-step down test measured physical performance.
Results
Adolescent psychological beliefs were significantly associated with pain (FABQ-PA r = 0.33, and PCS r = 0.34), function (FABQ-PA r = −0.59,TSK-11 r = −0.33), hip strength (FABQ-PA r = −0.41, TSK-11 r = −0.32), and single leg hop for distance (FABQ-PA r = −0.38). Parent psychological beliefs were not associated with the adolescents beliefs, pain or function.
Conclusions

Since amplified music gained widespread popularity, there has been community concern that leisure-noise exposure may cause hearing loss in adolescents and young adults who would otherwise be free from hearing impairment. Repeated exposure to personal stereo players and music events (e.g., nightclubbing, rock concerts, and music festivals) are of particular concern. The same attention has not been paid to leisure-noise exposure risks for young people with hearing impairment (either present from birth or acquired before adulthood). This article reports on the analysis of a subset of data (leisure participation measures) collected during a large, two-phase study of the hearing health, attitudes, and behaviors of 11- to 35-yr-old Australians conducted by the National Acoustic Laboratories (n = 1,667 hearing threshold level datasets analyzed).
