The topic of this work is the Osteochondral lesions of the Talar Dome (OLT).
The talus is the principal bone of the ankle, with the calcaneus they form the skeleton of the posterior half of the foot. Much of this bone is covered with cartilage, talus is essential to the mobility of the ankle. It is the articulation of the body which often suffers from lesions. Sportsmen are the most affected by these lesions. OLT is a transchondral fracture, a lesion of the talus cartilage located on the dome, frequently occur after traumatic injuries, up to 50% of patients with an ankle sprain while also develop a cartilage injury in the ankle-joint or osteochondral lesion of the talus. OLT is a source of persistent pain, joint cracking, increased ankle volume and chronic instability. In order to rapidly diagnose the pathology and to decide on the ideal treatment strategy, its essential to use the right modality of imaging. Occasionally, regular X-rays can show an OLT but frequently additional imaging is needed. In the osteoarticular area, MRI is in constant evolution. Thanks to its good sensitivity, MRI is considered as the modality of choice for the joints. Nevertheless, for the detection of osteochondral lesions of the talar dome, CT arthrography (CTA) is a well-established imaging technique because the articular cartilage at the ankle is very thin ( CTA is a two-step technique that requires first an intra-articular injection of iodinated contrast performed under RX guidance and precedes the CT imaging. In conclusion, this subject explains the importance and the comparison of different modalities to diagnose and to treat the OLT. CTA showed improved sensitivity and a good reability for the detection of osteochondral lesions at the ankle compared to MRI but its also helpful for the appropriate clinical context. |
