While some nausea and vomiting in pregnancy (NVP) is considered normal impacting around 70 per cent of all women hyperemesis gravidarum (HG) is at the severe end of the spectrum, affecting between 0.33.6 per cent (Einarson et al 2013).
The impact of HG on a woman's physical and mental health as well as that of her unborn baby can be devastating, ultimately leading to life-threatening complications (Fejzo et al 2019). Despite this, some health care professionals are failing to recognise the condition, and its impact on women and their babies, because of out-of-date beliefs regarding its definition, aetiology and treatment. Women with HG often report that their symptoms are not being taken seriously by medical staff, with a feeling of reluctance from doctors to prescribe medication. Worryingly, a recent survey found over 40 per cent of women suffering from HG had to request medication themselves from doctors (Nana et al 2021). Serious concerns have also been raised about the continued reliance on ketones as a diagnostic marker for HG, with alarming anecdotal evidence of dehydrated women being turned away from hospitals because they are unable to produce a urine sample to test for ketones (Dean 2018, Fejzo et al 2019). Clinicians' knowledge of HG needs to expand and midwives can play a vital role in both challenging out-of-date beliefs and improving the experiences of women with HG, by advocating for them and truly listening to what they have to say (Dean et al 2018, Nana et al 2021). |
