Lactational atrophic vaginitis (LAV) is a condition specific to breastfeeding women and is characterised by vaginal atrophy, caused by hypoestrogenism during lactation. This state, not dissimilar to menopause, leads to a presentation of decreased elasticity in the tissues, reduced glycogen content of the epithelium and reduced blood flow to the urogenital tissues (North American Menopause Society 2002). Evidence from Lev-Sagie et al (2020) reports a high prevalence of urogenital atrophy in breastfeeding women.
A fall in placental oestrogen, combined with the antagonistic effect of prolactin on oestrogen production when breastfeeding, causes a decline in systemic oestrogen post-childbirth. Although the primary aetiologies for the fall in oestrogen levels during menopause and LAV are different, the resulting signs, symptoms and clinical presentation are alike. The hypoestrogenic state results in reduced glycogen production, an increase in lactic acid and vaginal pH, and leaves tissues thinned, atrophic and more susceptible to damage and irritation (Kim et al 2015). As for menopause, women with LAV describe localised irritation of the genital tissues, dryness, itching, burning, reduced vaginal secretions during sexual intercourse and dyspareunia (National Institute for Health and Care Excellence (NICE) 2022). Clinical examination can reveal a lack of moisture, a reduction in tissue elasticity, pallor, erythema and petechiae (Kim et al 2015). |
