TITLE: The psychometric properties of the Leicester Cough Questionnaire: a systematic review.
OBJECTIVES: This study aims to provide a comprehensive overview of the LCQs measurement properties. METHOD: A search was conducted in PubMed, CINAHL, and ScienceDirect databases until October 1rst 2022. All full-text articles, published in French or English, aimed at evaluating the LCQs content validity or psychometric properties were included. Articles were screened independently by two reviewers for eligibility. The COSMIN Risk of Bias checklist was applied to assess the methodological quality and results. Results were qualitatively summarised and rated by a modified GRADE approach. RESULTS: 42 studies were included accounting for 8,794 mainly adult participants. Eligibility was based on the presence of a cough or respiratory condition. Chronic cough lasting more than 8 weeks was the most represented. This review found that the LCQ total score is relevant for the assessment of cough impact on QoL and comprehensible. The original 3-factor model showed a satisfactory model fit. Good convergent validity was found for the total and physical domain scores. The total and domain scores demonstrate good internal consistency and testretest reliability (except the physical domain). They are responsive to change following cough recovery or respiratory exacerbation. Estimates of MID thresholds were 1.7 and 0.4 for total and domain scores respectively. CONCLUSIONS: These conclusions were impaired by the poor methodological quality of included studies, and additional research is needed to assess its comprehensiveness, unidimensionality, cross-cultural validity, and measurement error of all scores, so as domain construct validity and test-retest reliability. Further assessment of its application to long-term monitoring of patients is advised for chronic conditions. High-quality studies in line with COSMIN standards, selecting samples for the presence of cough, so as equivalent assessments of alternative outcomes, would further support the LCQ's application in clinical trials and for the monitoring of coughers. |
