Purpose: The analysis of rejected and repeated acquisitions is an issue to be taken into consideration for the principle of optimization in radiation protection. Indeed they lead to unnecessary irradiation of the patient, cost and unnecessary occupation of the facilities. The objectives of our study are to investigate the rate of rejection and repetition, to identify their causes and to evaluate possible differences between observers in perception of the quality criteria required for an X-ray examination.
Materials and methods: DACS software allowed us to isolate examinations that might include rejected and repeated acquisitions. We then retrospectively analysed these data over a three-months period. We recruited 67 examinations for repeated acquisitions and 458 for rejections. We also submitted a questionnaire to radiographers and radiologists to assess the variability between observers on quality criteria for validating a chest bedside radiograph. We then statistically analysed the collected data. Results: The rates of rejection and repetition were respectively 4.03% and 4.31%. The Fleiss Kappa coefficient applied to responses to the questionnaire was 0.33 for the radiographers and 0.58 for the radiologists indicating low and moderate agreements respectively. Among acquisitions, we observed a small proportion of errors in the choice of tube, indicating no direct irradiation of the patients. Errors were more frequent for the more complicated examinations such as bedside chest radiographs or the lumbar spine. Conclusion: We describe a process for recruiting repeated and rejected acquisitions using a DACS. Our study shows a rejection rate within limits of that published earlier in the literature. Examples of repeated acquisitions are used to improve practices and reduce variabilities. The long-term impact of such quality control will require further studies |
