75% of Talar avascular necrosis (AVN) is associated with trauma, and in particular, talar neck fractures.
• Revascularization of the talus can take up to 36 months by creeping substitution. • Prolonged non–weight bearing, partial weight bearing, and bracing have been used for conservative treatment but have not been shown to prevent collapse or improve outcomes. • Tibiotalar arthrodesis, subtalar arthrodesis, and tibiotalocalcaneal arthrodesis are all potential options for treating late-stage talar AVN based on patient examination, imaging, and selective joint injections. • Principles of arthrodesis include the removal of any necrotic bone; the addition of autograft, allograft, and various biologic augments for incorporation; and instrumentation that maintains appropriate rigidity, compression, and alignment. |
