Avascular necrosis following rotational ankle fractures is most commonly described in the talus; however, it can also occur in the tibial plafond. These sequelae of ankle fractures are rarely described in the literature.
• Avascular necrosis of the distal tibia after rotational ankle fractures must be included in the differential diagnosis in patients who continue to have pain despite radiographic healing of the fractures. • Diagnosis of avascular necrosis of the distal tibia is best confirmed with MRI of the involved extremity. • Treatment options for this condition include observation, limited weight-bearing, bisphoshonates, hyperbaric oxygen therapy, shock wave therapy, pulsed wave electromagnetic field therapy and physical therapy, and surgical options such as percutaneous drilling, debridement with bone grafting (vascularized and nonvascularized), arthrodesis, and total ankle arthroplasty. |
