Core decompression is a relatively easy procedure from a technical standpoint and the results demonstrated it can be used with success to treat patients with early stage osteonecrosis of the talus.
Percutaneous technique has also shown success and can be particularly useful in patients with polyarticular involvement who may have to undergo more invasive procedures for osteonecrosis of the hip, the knee, or other joints. Data on bone grafting are lacking, but it has been used mainly to treat traumatic patients who may benefit from augmenting the bony stalk. Vascularized grafts were the most commonly reported techniques in the literature but, because of their technical difficulty, it may not reflect a true estimate of their use and actual performance. Larger, prospective, multicenter studies are needed to guide specific patient selection criteria for each surgical intervention option. |
