The Ficat classification describes the current understanding of the natural history of avascular necrosis (AVN) of the talus, including preclinical, preradiographic, precollapse, postcollapse, and arthritic stages, though patients may remain at a stage for long periods of time.
The natural history of patients with preradiographic or precollapse AVN likely depends on the size and location of the lesion. Symptoms do not correlate with stage, and patients may be minimally symptomatic despite diffuse involvement for long periods of time. Joint-sparing treatments hold promise to prevent collapse but none are universally successful. When structural collapse occurs, joint-sacrificing procedures may be needed to relieve symptoms. |
