The K-wire for hammertoe deformity correction was first proposed and utilized in 1940 by Taylor,5 and for almost 80 years it has remained the most commonly recognized and universally used implant for digital deformity correction. However, there have been exciting advances in technology over the last 35 years with the advent of intramedullary implants of various composition and architectural structures. The authors believe all categories of fixation method have the potential to achieve a satisfactory result, if used appropriately. Although research is still lacking when it comes to clinical trials, many of these devices stand out with strong case studies. Digital correction, similar to other surgical procedures, should include careful selection. One must place both the surgeons experience and the patients clinical picture and personal goals into consideration. Although authors of this article utilize K-wire fixation as the main course of treatment for hammertoe deformity correction, they recommend use of intramedullary fixation in cases of arthrodesis revision, in patients whom there is increased risk for pin tract infections, or for patients who might be a poor candidate for K-wire fixation.
There is continuous evolution, ongoing modifications, and advances in digital implants with aim at reducing postoperative complications in deformity correction. Further evaluation and comparative studies are needed to determine the cost-effectiveness of the new devices in addition to outcomes. Despite much research being done in engineering, few randomized controlled trials exist in use and application of implantable devices. |
